Your stay
Various modes of hospitalization exist.
Inpatients stay several days, admissions are scheduled from 3 pm till 5 pm.
Outpatients stay from a few hours to a day : your admission time will be specified to you by your doctor. The hour of admission varies between 7 am and 2 pm.
For any scheduled admission :
If you must have surgery, a consultation with an anesthesiologist is required. It will take place in our facility by our anesthesologists.
It is important to :
- Settle an appointment : 03 83 18 83 20.
- Don't forget to bring your medical prescription.
- You will then be welcomed by our administrative hostesses who will fill the necessary paperwork for your future admission. You can then specify your wish concerning a single room
NB: single room can only be attributed according to available rooms.
The admission as a matter of urgency :
You can be admitted at any time of day or night, without prior administrative formalities, and with the agreement of the doctor working at the clinic and to whom you have been sent. The person accompanying you will file administrative paperwork needed for your hospitalization at the admission desk during open hours.
Formalities and tips
On the day of your Admission, come to the admission desk to complete and sign the remaining paperwork and provide the following documents :
Medical documents :
- Your Health Book
- Your blood group card
- Blood tests, X-ray, electrocardiogram ...
- The list of medications you are taking.
Administrative documents :
- Your vital card
- Your ID
- Your CMU certificate if it applies to you
- Your health insurance form or card
You must also provide given your situation :
- Strand 2 of the Declaration of work incident issued by the employer
- The notification of the AES (Special Education Allowance)
- The book of free treatment for war pensioners (art 115)
If you are unemployed :
- ASSEDIC notification and pre payment documents
If you are itinerant or without fixed address :
- The circulation book
If you are a foreigner :
- Your residence card or VISA
- The documents mentioning the terms of your health insurance (E111 if you live in the European Union or SE 352 for Algerians)
Discharge
The date of your discharge is communicated to you by your doctor and is determined according to your health. The admission desk is open since 10 am to settle the last formalities.
The necessary paperwork to provide to your employer :
- your insurance company
- your invoice
- the additional costs of your stay (telephone and television for example)
The nurses will give you any document necessary for your medical follow-up (prescriptions, X-ray, etc).
However, if you wished to leave before this date, without preliminary agreement, we will ask you to sign a discharge form.
